The Program Manager will oversee the administrative operations of the Health Home Program, as well as supervising the Outreach and Enrollment Team. In this role, the Program Manager will oversee personnel and resources at multiple sites to improve quality, workflows and efficiency, which ultimately supports the goals and objectives of the Health Home Program.
Master's Degree required (MSW MHC, MSN, MPH, or any health-related field). Clinical license in stated field preferred. Project or Change Management and Administrative experience in a health care or social service setting.
Strong administrative and clinical background with expertise in excel, online databases, change management and performance improvement. Competency in Microsoft products and electronic databases. Must have experience with policy integration in developing and maintaining program operations and managing personnel. Must be organized, an excellent communicator and able to manage multiple and sometimes competing priorities.
